Lumpectomy: exercise, OT etc.. before surgery ?

I have completed neoadjuvant chemotherapy (July-Dec 2019) and I'm scheduled for a lumpectomy next week and most likely full axilla clearance. There was no advice from my doctor regarding seeing an occupational therapist or physiotherapist prior to surgery.  I would like to find out what others have done prior to surgery to improve outcomes and avoid lymphedema, cording etc..
  • I had major surgery,  non BC, last year.   As part of my preparation I had a lymphatic drainage massage.  As the Anaesthetist prepped me I mentioned it and he was surprised.  It paid off as the drain tube was in for 12 hours only.

    I did things the other way.  Surgery first.  I say physio for base line a few days before surgery but that was it.  I had the lumpectomy and full ax clearance.  I had minor cording afterwards but a couple of physio sessions sorted that out.  It is very important that you do the exercises.  I found the physio quite painful the first week but improved quickly.   Honestly the more you can do the better, especially the wall walk and the ones above your head. 
    I've never had a problem with lymphodema and had full strength back in that arm within three of four months.

  • Hi @lady_with_green_eyes. Like you, I had neoadjuvant chemo. The onky thing my bc nurse did for me in that  time was suggest seeing a lymphoedema therapist before surgery.  I was one of the lucky ones 😛. I developed lymphoedema 6 weeks afyer surgery.  Because we had pre-op measurements to compare,  we knrw I needed to start treatment, so I  went in to my first compression garment that day. In part because of that early diagnosis (& maybe a bit of luck) I've been pretty stable, and never even had cellutis.

    Hope you can get in to someone before your surgery.  They can be hard to get in to

  • With hindsight, I would have spoken to a good lymphoedema therapist before surgery (as I knew I had a number of suspicious looking lymph nodes which would be removed - all but one were benign) about minimising risk. I did all the recommended exercises religiously, but didn’t notice anything until I realised my watch band was too tight! It may be worthwhile to be prepared! I do have lymphoedema but with a good therapist, it’s quite manageable. Best wishes. 
  • @lady_with_green_eyes I had a lymphodema consult at my hospital before lumpectomy surgery to take baseline measurements and speak to the therapist about avoiding risks. I just went back the other day after completing treatment and had a special massage where she showed me how to clear fluid from the breast towards the other side. I only had two lymph nodes removed but had radiation to that side as well and extra to some other lymph nodes. I've had no symptoms but I think being proactive is good. I didn't have a clearance as you think you will.
    Its good to speak to a professional and good to have the measurements from before so you can compare and jump on to things if they start to change but too subtle to notice (that's what they said too.) Good luck with it!
