Topical Cream use on sore hands/thumbs from Letrozole/Tamoxifen/Exemestane
Hi guys
As with many of you - i've had extremely sore hands (and thumbs in particular) since starting Letrozole and then changing to Exemestane 6 months ago. Initially, the pain was manageable - but just now, I can't even use tongs to put food onto a plate! It is quite debilitating.
It is now 24/7 pain and become worse in the last 2 weeks. I am aware of whenever I awake during the night - it is a continual pain/ache/numbness ..... and worst of all, it is now affecting my ukulele playing & even my fishing! :( My fingers feel like fat sausages (swollen) and now don't bend well and are not good at holding things - I can't form a fist anymore! :(
SO ..... when in NZ a few years back, I grabbed some Sports Liniment Cream developed by one of NZ's best Sport's Injury Specialist, caller Percutane! It is available in Aussie at Amcal Chemists, I believe. It is similar to Voltaren.
I put some over my whole hands/fingers this morning and am already feeling the results - I've rubbed my nose & my lips & they are burning like hell!! So if it gets thru my skin & attacks the pain areas like it has there already ..... I am hoping it will work wonders!!
Some info on it below (I don't have any affiliation with the product - but have used it previously for shoulder pain & it worked.)
I'll just have to be careful when I go to the loo!!

How Does Percutane Pain Relief Cream Work?
Percutane Pain Relief Cream contains four key ingredients that together help soothe pain and discomfort in muscles and joints

Capsaicin (Capsicum fruit oleoresin) is a substance derived from the seeds and membranes of certain plants that belong to the nightshade family, such as chillies and capsicum.11,12 It is used, among other things, to help relieve muscle and arthritic pain.11 It is believed that capsaicin reduces pain by temporarily reducing levels of the neurotransmitter, substance P, which relays pain signals to the brain.12-14
Neurotransmitters are chemicals that help communicate information between nerve cells. There are numerous clinical trials that have been performed that show Capsaicin is very effective at providing pain relief.

Arnica: Arnica (Arnica montana) is a herb that has been used for medicinal purposes since the 1500s. The fresh or dried flower heads are used in creams to soothe muscle aches, reduce inflammation, and heal wounds.1 The anti-inflammatory effects of arnica are thought to be due to one of the compounds it contains.2 In a clinical study, arnica was shown to improve pain, stiffness, and function in people with osteoarthritis of the knee.3 Another study showed that arnica was as effective as ibuprofen in reducing pain and increasing function in patients with osteoarthritis of the hand.4

Burdock: Burdock (Arctium lappa) is a member of the daisy family and has been used for centuries to treat a range of ailments.8 The seeds of the plant contain compounds that have anti-inflammatory effects.9 Burdock is also believed to be useful in treating a number of conditions including arthritis and skin problems.10

Aloe vera: The aloe vera plant has been used for thousands of years to treat a number of conditions.5 These include wound healing and inflammation.6 Aloe gel contains substances known as glycoproteins and polysaccharides. Glycoproteins speed the healing process by stopping pain and inflammation, while polysaccharides stimulate skin growth and repair.5

How do these ingredients help Arthritis Sufferers?
Substance P is involved in the development of and inflammation associated with arthritis.14 Studies have shown that topical capsaicin is effective in reducing pain in osteoarthritis and rheumatoid arthritis sufferers. In one study, capsaicin substantially reduced tenderness and pain in people with painful osteoarthritis of the hand.15 In another study, after 4 weeks of application, capsaicin reduced pain in people with osteoarthritis of the knee by 57% and in people with rheumatoid arthritis by 33%.14
How do these ingredients help neuralgia and Post Herpetic Neuralgia (PHN) sufferers?
Percutane gives relief of nerve pain/mild neuralgia and the pain of shingles rash and PHN by topical application (to be used only on unbroken skin). It provides temporary relief of nerve pain/ mild neuralgia by inhibiting Substance P, which transmits pain signals to the brain.
Substance P is thought to contribute to certain types of skin pain. Because this pain is similar to that associated with PHN, researchers have investigated whether capsaicin is effective in treating PHN.13
One study looked at people who had had PHN pain for 6 months or longer and found that after 6 weeks of application of topical capsaicin, pain and the ability to function were significantly improved. In addition, the improvements continued in 86% of people who kept applying capsaicin for up to 2 years.12 In another study, significantly more people with PHN treated with capsaicin experienced relief from pain compared with those who used a placebo cream. After 6 weeks, almost 80% of people experienced some relief from their pain.13
How do these ingredients help Fibromyalgia sufferers?
Fibromyalgia is a condition associated with chronic musculoskeletal pain. The majority of people affected are young women in their 20s’ or 30s’. It’s not known what causes the pain, but one theory suggests that substance P may be involved.16 In one study, people with fibromyalgia who applied capsaicin for 4 weeks on tender points reported less tenderness than those who applied a placebo cream. After 2 weeks, people who applied capsaicin to their hands had a significant improvement in grip strength.16

- FlaneuseMember@Deanne @kmakm I've been off Letrozole for 23 days. My hand and feet pain started lessening about six days ago, for a couple of days, but for the past three days my hands have been very painful. I had underlying arthritis anyway. I suspect the Letrozole has made it worse; my lymphoedema physio says that happens. My oncologist said on Monday that it can take about a month for the effects of Letrozole to wear off. Maybe because I'm old and because of the arthritis, it could be worse too.
- kmakmMember@Deanne When I had my Letrozole holiday it took about two and a half weeks for the pain in my thumbs to stop.
- FlaneuseMember@arpie A friend just gave me a tub of Pain Away for my hands. If it doesn't work I'll try your wonder cream. BTW, I use lemon juice after chopping chilli. I keep a couple of squeezed lemon halves in a ziplock bag in the fridge and rub my fingers inside them. I throw them away after a few days.
- DeanneMemberSo glad that the prednisone worked for you and allowed you to enjoy the festival. My right thumb is just stiff and I cannot bend it at all. Makes it hard to hold anything. As a teacher it is affecting my ability to work.
Today, after the patch, I can bend it but it is clunky in the joint and some pain is now starting to happen. I think the patch has worn off now. I am hopeful that being off Letrozole and having the acupuncture will lead to a permanent fix but the patches might help to speed things along and give me some ‘normal’ movement in the mean time. I will try one again tonight. - arpieMemberYes, thank you - my hands behaved themselves for the Uke Festval - The prednisone I took a week before & up tothe Uke Festival worked really well. I stopped taking it last Sun - and my hands are still relatively 'ache free' - altho the thumbs remain sore. Normally, I can't close my hands at all in the morning (they look like I am holding an invisible apple.) But just now, I can close both to a fist still!!
Oooh - those patches sound interesting @Deanne ! Hopefully I will find some in Sydney next week when down for my 1st year review with my surgeon! (or will try for them online!)

Do your thumbs 'lock' when you close them? My left one does! Quite weird.
Wow, they must be super strong! Maybe just try it for an hour at a time .... you'd probably be able to even 're-use' that first bit that you took off after the 2hrs? I am sure there'd be enough active ingredient in it!Yes, I must admit, when using it, I scratched a couple of 'tender spots' and inadvertently transferred some Percutane to it & it stung like hell!! I CAN recommend it for sore muscles/joints, tho - having used it often on my shoulders when I had a totally stuffed shoulder a few years back (still recommending gloves or a plastic bag to apply!!) - DeanneMemberHey @arpie, I hope your thumbs behaved themselves for your Uke festival. I have started having acupuncture for my thumb. This week they gave me a couple of patches containing capsaicin to try. Tried one out last night, cutting it up and wrapping it around the painful parts. I could only stand it for 2 hours and they had to come off, but my thumb is bending without pain today.
I can still feel the heat doing it’s thing even though I took the patch pieces off over 12 hours ago. I found the patch much less hazardous to use because it did not get all over my hands like when you apply the cream. Will be interesting to see how long the effects last. Hopefully between stopping Letrozole and changing back to Tamoxifen, the acupuncture, and the patches, I will get my thumb working properly again soon. I think the patches are available from Asian grocers. I found some online and they are not very expensive. They are called Astrobon Astrapharm Chille patches. - arpieMemberThat's wonderful, @Deanne --- It is highly unlikely to be tendonitis .... and highly likely to be from your Hormone Inhibitor tablets ( are you on these?!) If it works, great! I was using it on other aches/muscle issues before I was diagnosed with BC ..... so knows it works!! ;)
I have fished today in my kayak for nearly 8hrs ..... and barely noticed my hands!! (I tested my hand and made a fist a couple of times & it worked both times!) Given that I need both hands to work efficiently when casting AND retrieving .... I am very happy with today's outing! (Only happier if more fish landed! LOL)
I will stick with the night time routine as it is very much less likely for me to 'sting myself' overnight!
Percutane is definitely 'worth a try' if you are having extreme hand/thumb pain from your Hormone inhibitors ..... give it a go! ;) xxx - DeanneMemberIt is definitely helping my painful thumb @arpie. I put it on my right thumb down to the wrist and in between it and the pointer finger. Also on the palm where it is very painful at the base of the thumb. I use my other hand to apply it, avoiding using the pointer finger which I am most likely to use to touch somewhere else and wash it off as soon as possible!
The GP has me on Nurofen for 3 days and thinks it is just tendinitis from overuse. Had been doing lots of wheelbarrowing of mulch! It is frustrating how easily I seem to injure myself these days. I am supposed to rest it and avoid lifting anything heavy for a week or two. Pretty difficult as nearly everything I go to do hurts it but hopefully it gets better soon. Thanks for the Percutane tip. It is helping! :) - arpieMemberOK .... So i've given the plastic bag away and now use some folded up tissue to put the cream on!!
I put it all over the back of my hand, then all around the base of my thumb and back up between my thumb & forefinger. So far, I haven't stung myself again!this morning, tho still painful, I can form a fist with my left hand!! So happy with that!! I will keep going - just at night at this point in time .... it gets a bit greasy for my day time use .... and see how I go. - Annie_CMember@arpie and others
After chopping chillies for my chilli chutney (yum) I rinse my hands in milk. For some reason milk seems to remove chilli better than soap and water. Unfortunate side effect is that you do acquire a "dairy" aroma!
As this product is capsaicin based (chillies) it should work.
My pharmacist has ordered some in for me.
