Heart Palpitations without chemo - could it be a side effect of AIs?

Hi guys

Has anyone else had heart palpitations who HASN'T been on chemo, but IS on AIs?  I've been on Arimidex for 12 months now.  I guess it would be similar to 'panic attacks'?  Tho pretty sure it isn't this!

I've had a scary few days this week, with thumping heart palpitations (skipping beats every 5 or 6 beats, then maybe after 10-15 beats) but yesterday morning at 3am, it was after EVERY beat & it scared the crap out of me.  I also had a 'throat blockage' that felt a bit like indigestion & I know this can be a sign of a heart attack, but didn't have the pressure, pain or clamminess associated with a heart attack. I was awake from 3am on ..... So I 'sat it out' til 10am with the thumping skipped beats still happening - then went to the GP. They did an ECG but it didn't show up - and wanted bloods done to make sure the troponin levels weren't 'up' (showing an enzyme related to heart attacks) but there were no pathology centres open on Sat in our towns which surprised me as they always used to be open - maybe they closed due to Covid?  So now I had to go to the closest hospital (and waste THEIR time) to get a simple blood test, ECG etc, when the pathology place should have been open.  :(

I said I would drive to the closest town where the hospital would do the bloods - but they didn't want me to drive in case I had an episode on the way ... so they called the ambos to take me.  This really upset me as it is a waste of an emergency service & I felt guilty that I was using the Ambulance when a more serious case could be impacted by it not being available!  :(

So at the hospital, they did another ECG (the beat abnormality didn't show, even tho the skipping continued throughout the day.)  Chest xray was clear.  Bloods were good.  I finally got  home at 6pm with one of my uke ladies driving up to pick me up.  Hubby thought I'd been out kayak fishing all day!    His dementia at least stopped him being worried! LOL

I go back to the GP for more bloods tomorrow (thyroid etc) and hopefully get fitted with a Holter monitor to check my heart beat over the next day or so - and I truly hope that it will pick up on the skipped beats.  My dad died young from heart (had his first one at 40 and died at 50 with numerous attacks in between.)  My brother was fitted with a pacemaker 2 years ago after a single catastrophic incident (he is incredibly fit & thin so that was a real surprise!)  My sister now tells me she has had palpitations for years - I didn't know!  So it is possibly inherited, could be age related, or could be a side effect from my AI tablets! 

I see my Onc on Tues, so will be mentioning both the ticker problems & the urinary urgency (which started about 4-6 weeks ago) as my main concerns.

Who else has had heart issues when they've NOT been on chemo but HAVE been on AIs?  (I know that chemo can cause heart problems - and they now check patients before & after chemo - tho the problems can start years after the treatment - so everyone needs to be aware of this.)

Hopefully this will all settle down soon without the need for too much medical intervention - I DO need to drink more water, as this can upset the electrolyte balance in the body (as can drinking TOO much water - it washes the electrolytes OUT of the body. This happened to my neighbour - she ended up with a pacemaker.)  So I have my fingers crossed.

It has also raised fears about what will happen to my husband if something adverse DOES happen to me - as we don't have family nearby - and I can't rely on friends to take over the care of hubby (with dementia) if I am unable to do so.  I will be contacting My Aged Care this week to kick start getting him 'on the books' even if we don't use their services initially.

What sort of Plan B do you have for your elderly partner/parents, in case something happens to you?  We don't have children tho we do have 2 stepchildren in NZ - but can't expect them to take over his care. 
  • OK - so I've got the Holter Monitor on for 24hrs .... and I reckon there should be heaps of missed beats showing up!!  I am Trying to document all the 'hits' - I may need a new page!  If it helps with a diagnosis, it will be good, specially if we can STOP the bloody blips from happening!  Will be arranging an appt with a cardiologist some time soon.  

    Got my Onc appointment tomorrow by phone now so at least I don't have to travel 1/2hr ech way to get to it - as there is always a lengthy wait to get in & it would have interfered with returning the monitor in time = more stress!  

    Bloods were all good, so not thyroid.
  • Thanks for your concerns @lrb_03 & @Dory65 - I hope I get answers & solution too. I will definitely call an ambulance if it gets really bad again like yesterday morning.  I just hope it settles down - Today, I've been able to record video of my blood pressure machine that shows the heart beat & I have a few showing a skipped beat, as well as one doing a 'triple beat' ..... so will show the GP tomorrow - but it still needs to be 'seen' on 'their' machine, to be believed, I fear

    I've heard that the Holter monitor does not always pick it up too @ddon   :(   That is a real worry.  Your description echoes my own, so clearly!  :( 

    Apparently thyroid conditions can cause it as well - not that I am aware of having a thyroid condition - but blood tests tomorrow will check it.  Apparently a lot of women have over or under active thyroids.  My BC surgeon was also a specialist in thyroid .... which I found very interesting!
  • Can I recommend calling an ambulance if you get those more severe "runs" of palpitations? It's not a wadte of emergency services time, and could save your life.
    In the meantime, continue to to follow up with your GP , please. Your life is valuable 
  • Mine showed up on ECGs and monitor in hospital but zip on the halter. By the time I was in hospital they had settled to be infrequent but I had days at home where it just went on and on. I could see my chest wall bouncing. Awful.

  • Thanks for the reassurance @ddon and @Afraser  -  it sure is weird when it starts!!  I had a great sleep last night & felt 100% earlier today - I've had a couple of skipped beats just now ... but not overly concerned about them & know that it can't kill me - with the worst thing causing a clot & maybe a stroke tho. 

    haha @ddon  - I've had 3 ECGs already & it isn't showing on any of them!  So just hope it does on the monitor - so it doesn't look like I am making it up!  LOL  I've always considered myself to be fairly bullet proof, so the BC diagnosis was a big hit to my self belief .... and probably why this sudden occurrence has also hit me for a 6!!

    Fingers crossed I get some answers next week.  I think I'd been kayaking on the same day as the initial occurrences - so may need a stress test too.  Last one I had was 6 years ago when I had pericarditis (inflammation of the lining of the heart) which presented itself with massive chest pains - and I thought I WAS having a heart attack!!  :(  Angiogram showed arteries were clear back then tho.
  • That is pretty scary,  I know. Exactly what my heart was doing- but it was chemo induced. Those are ‘extra beats’ or ectopic beats by what you are describing ( I was an RN and worked years in coronary care) and they are usually harmless but can be uncomfortable. A halter can pick them up but they are notorious for stopping for the 24 hours the monitor is on. I take beta blocking medication now which helps enormously. I hope that’s all it turns out to be for you - I hear your worry for your husband. It’s awful to be afraid for your health because of the need to be the carer for partners or kids. 
  • Hope your tests clarify the situation @arpie! I was diagnosed with an arrhythmia about two thirds through chemo - but of course it’s a condition that can occur all in its own. Classic symptoms are palpitations and a sort of ‘fight or flight’ response, like an adrenaline rush. I didn’t know because I am virtually asymptomatic - I’ve had palpitations twice in seven years, I have to take my pulse to know what my heartbeat is doing. When first discovered, my surgeon cheerfully described my heartbeat as ‘all over the shop’.
    Happily it responds well to treatment -  a cardioversion ‘fixed’ it for a year or so, as did medication but it seems to be slowly fixing itself. I now take beta blockers only to help regulate rate and my heart is in normal sinus rhythm for considerable periods, then has an occasional blip. Doesn’t affect anything much otherwise. Hope it all clears up. Best wishes.
