Sounds like a grumpy old doc you are seeing. I put cream all over my breasts, nipple, under arm and anywhere the radiation would be zapping. It is to moisturise so it can't possibly make them crusty. Your nipple will be sore from the radiation you poor luv so just be as gentle as you can.
My nipple still gets crusty sometimes so I tend to put bio oil on it now at the same time I putting it on my scars.
I just used a perfume free sorbolene given to me by the rads department.
I am hoping you get a nice nurse who is a bit more sympathetic and understanding.
