Not sure why you seem to be geting put through all this. What state are you in and is it the same one where this all occurred????
I am in Queensland and am in the process of having genetic testing done - simply took a referral from my GP to Queensland Health (I did find and download a form for him) - after the question was raised and agreed to by 3 specialists I still see.
The referral was sent in, I have had an inital phone interview (asking what I want to get from this and why I want to have it done) and paperwork (including permission forms for collecting more medical info on me and family cancer history) has been completed and sent back. Now just waiting for an actual appointment date.
I will keep you informed - puzzling why things should be different - we all have the same concerns.
