At my department inflammatory is treated with the same regime as other breasts (16 or 25 treatments depending on if nodes are treated). We only treat twice a day if the patients needs to finish by a certain date, certainly not for the whole course. 6 hours is the standard gap to give healthy cells time to recover, never heard of 4. But with clinical trials and such 'standards' are being updated all the time.
