Travel rebate

Hi all I am a newby! I live in country Vic and will be travelling to Melbourne for my chemo I was told I could get travel rebate? how do I apply? thanks in advance

    Hello Debra

    If you go to "www2.health.vic.gov.au/hospitals-and-health-services/rural-health/vptas-how-to-apply" you will be able to read all about the Victorian Patient Transport Assistance Scheme.

    That page also has the form which you can download.

    This may save you wandering around trying to get the correct information.

    Hi Debra, and welcome to the network. I also live in country Vic, and got my travel reimbursement forms from my local health centre. My breast care nurse had some forms too. Just ask your breast care nurse, she should be able to advise you how to access them.
