In Isolation with neutropenia in TCH Chemo Cycle 1

I apologise to those I was chatting with last week: I just  vanished off the face of the earth for the past week, but my white blood cell count became severely neutripenic, and then dropped very scarily low, ie down to 0.11 which means I have only 110 neutrophils where healthy people have about 4000. I was told to avoid any possible sources of information including telephones and my dirty laptop and to avoid sharing air with anyone who might have a cold or other infection. My throat and mouth have been tender and mildly sore so I could not even skype people, and I have had a temp of 37.1, and keep ringing the lovely oncology nurses in Mount Chemo lounge for info and guidance.

I understand that if my neutrophil was  0.5 last Sat, and 0.11 on Tuesday, it well may be a problem at least until today or even longer. I just hope it has come back up, and wish I had some way to check. I was told they trial us all to see how we cope, and because I got severe neutropenia, I will now get the Neulasta injection 24 hours after my next round of Chemotherapy and herceptin. This in turn causes severe bone pain, and they recommend NSAIDs like ibrufen for it, but I cannot take NSAIDs. I have also read a lot of people in the USA said the NSAIDs did not help much, but that Claritin, also known as Loratidine, a tricyclic antihistamine, does a really great job of minimising the bone pain.

I have also heard of the oncologist delaying the next Chemo cycle by a week or more if I don't come right up to the healthy level in time.

Does anyone have any helpful experience/comments to offer about Neulasta and non NSAID remedies, and about neutropenia, etc? Your help much appreciated.

    You will probably just have to wait and see how your own body reacts to the Neulasta.....I had no reaction worth mentioning....only had slight calf pain that felt like how your muscles feel day after a hard walk; but no bone pain or any other issues.

    (Oh, and had 3rd round of chemo delayed for a week till the blood levels came back up...but it was not a problem)...

  • I was told my white cell count went to zero....and I was frantic, taking my temp sometimes every 5 mins because my daughter had come home from school with a sore throat and I started to get one...i had people on standby to look after her and take me into hospital...but we were careful with hand hygiene and have bottles of hand gel around the place...anyway all went well and my next pre chemo blood tests were last tuesday and all were ok and was able to have the 2nd round of docetaxel and cyclophosphamide on wednesday. I had the neulasta yesterday, and I feel pretty good today, much better than day 3 of the first cycle, so it'll be interesting to see how things pan out over the next few days...I know i am less anxious this time around and will take pain meds more regularly. I have one of those dawn fraser circulation boosters and I used that last night because of painful legs I think from swelling...and that was helpful, so may used that a bit more this cycle.

    This feels so scary dosen't it.  I had similar drastic drops in neurtophils.  Neulasta did help but I had a few cycles delayed due to severe drops like yours so I ended up having to have neupogen injections as well.   Both Neulasta and Neupogen gave me bone pain.  I occasionally needed Endone so I'd encourage you to get a prescription in case. LIke you I can't take NSAIDS.  I did take Claratyn everyday for the last 3 months - hard to tell if that helped reduce the pain intensity.  My pain would often come in waves so once I got use to it I could breath through it - a bit like labour pain.  I was lucky and despite low neutrophils never had a fever or infection.  I did isolate myself avoiding crowds etc.  I did use my own computer and phone though.  Good luck I finally finished all my chemo rounds despite 7 weeks of delays due to low counts.  I know you'll get there too.

  • Hi Jessica , Sorry to hear that you are having troubles with your cell count. Mine did exactly the same thing and I had the injection as well . As with all treatment we all react so differently . I seemed to cop many side effects from the chemo but found I had no side effects at all with the injection . Hoping that will be you too. Take care, Sophie xx

  • So sorry to hear your neutraphil count dropped so low.  I was always told if mine dropped below 1.0 chemo would be stopped, but it only happened once and then came up quite quickly again.  I hope yours does, too.

    I had Loratadine an hour before every chemo, along with the standard Navoban and Dexamethsone, and it must have worked because I only needed Panadol a few times afterwards.  (My chemo was Taxol and Carboplatin weekly for 12 weeks.)  Deanne's experience with Neulasta is helpful - I didn't have it so nothing useful to add.
