choosing reconstruction after chemo and complications.

Hi everyone, please excuse if you've seen this in choosing reconstruction. Cosette suggested a wider audience here.
Any wisdom and experiences appreciated.
I'm in the middle of chemo right now.
Diagnosed August - carcinoma of (L) breast with pagets, stage 2, grade 3, 2 sentinel nodes (-), Her2 +.
Had (L) skin sparing mastectomy (nipple gone) with immediate reconstruction with tissue expander and intention to follow with implant and reduction of (R) breast for symmetry.
I had complications with continual Seroma and ++ drainage weekly.
Had top ups in expander and then 6 weeks post surgery and in week 2 of AC, low immunity, had cellulitis of breast, Seroma, and hospitalised for a week on IV AB's. My plastic surgeon and Oncologist suggested back to surgery for a cleanse and possible removal of expander depending on what presented at the time.
As it turned out, expander was removed. Swabs were NAD.
i am week 4 of Taxol and Herceptin now and have just had a change of plan.
Decided to move ahead - 8 weeks after chemo with (R) prophylactic mastectomy also, due to grade pathology and wanting to remove as much risk as possible, with bilateral reco with expanders for future implants.
l'm fretting that I still have some tethering of the (L) site skin pouch to my chest wall since expander removal and hope that will not affect the outcome.
My surgeon also suggested possibility of skipping the expanders and going ahead with the implants, given skin spare.?
Would like to hear if anyone else has encountered any of these complications and how they have been able to continue on and reach the 2 new breast goal?
Thanks all for sharing too.
Great support here.
A.

  • Thanks Cath.
    That's really good to hear.
You will have a matching set in no time.
    Perseverance is key.
And I won't be in a hurry to get the drains out, I think.
    Good luck with your surgery.

  • Hi Amazonia, I haven't had any of the complications that you mention but I did have a skin & nipple sparing mastectomy with immediate reconstruction using silicon implant (no expander). I didn't have any issues with this apart from my drains. The drains were to be removed after two consecutive days at less than 20mls - to stop seroma's occurring. I was in hospital for 6 days and my first drain was removed 4 days after that. Second drain hung on for 8 weeks and was removed due to levels not falling. Surgeon said she thought that the drain itself was causing irritation. As far as major tightness due to the implant I didn't really have any pain associated with immediate implant possibly a very slight tightness but nothing that caused me any issues. My surgeon actually apologised before the surgery and said she had measured me incorrectly and that I was in fact going up a size - I didn't have a problem with that either. So my experience is all good. The implant is now 6 months down the track - looks fantastic and has softened incredibly and now feels like a breast. I am looking at having surgery to match my other breast to my new anti-aging anti-sagger  and surgeon has said that is likely to be a smaller implant with the possibility of a reduction to match the perkiness :) Too good!!! If I can help you in any way let me know. Wishing you all the best. Xx Cath
