Diep Flap
I wanted to post a positive experience about having a reconstruction done via diep flap. I had a lumpectomy where the lower portion - a good portion - of my right breast was removed in April 2010. My breast cancer was diagnosed early but the surgeon took a wide incision and my breast ended up looking like a capsicum - or so my girls tell me. After four lots of chemo and radiation, I decided to have my breast reconstructed. It was not an easy decision as I was tormented by the thought people would think I was vain. I showed my breast to those who mattered and everyone who saw it said go right ahead.
Then I made the mistake of consulting Dr Google. Pretty well everything I read was a horror story. Barbaric was the word that come up pretty frequently. I saw my plastic surgeon, a great calm level headed gorgeous guy, Gerard Bayley in Brisbane. He calmly stated that none of his patients had written on the internet. He told me very matter of factly that the first day I would feel like I had been hit by a truck, the second day by a car and the third day I would be on the road to recovery. I got measured for a corset and bra. He explained the procedure. I had a couple of xrays and it seemed as if there were valid blood vessels to use for the removal of the flap and insertion into my breast.
The next few weeks I must admit I was extremely nervous. Very worried about what was ahead. I was having the op in early December and had a frantic rush to get Christmas presents organised etc. My husband's birthday is on December 24 and my daughter on December 28 so I also wanted to get that organised as well as I imagined myself as totally incapacitated for months.
The op commenced with Dr Bayley drawing on my stomach above my belly button and down to the pubic line. I said what's that and he told me that was what he would take out. Had a long talk with the anaethesist and it would be a long op. Seven and a half hours later I woke up. No pain really as that is well controlled. I had my corset on and a special dressing on my breast. I didn't have a need at that stage to look at it. I thought I would spend a couple of days in the ICU but instead went straight to the ward as the op had been very successful.
For the next two days the flap had to be checked every hour. It was hard. You basically doze all day and night. I got very uncomfortable in the bed and actually slept sitting up for two nights. Lots of meds and injections. A self adminstered pump for the pain. I had a reaction to morphine - vomiting - not pleasant with a dissected stomach muscle and a cut from hipbone to hipbone. The physio came on the day after. It really was about 12 hours after the op to get me out of bed. He took great care in the preparation of getting me up. I must admit I didn't mind getting out of bed. It wasn't nice cause I was on the "hit by the truck" day but I knew it had to be done.
The second day after was alright but I was still very uncomfortable in the bed. The flap was taking and I just kept thinking, "This will all be over soon". The third day I hit a wall. My IV feed failed and I had to go to Xray and get a pic line installed. Not what I felt like doing. I was upset and knew without a shadow of a doubt I would never be out of hospital. Through all this my husband and family were amazing. They were there for me all the time. I needed that this day.
On the fourth day, a wonderful nurse came to me early in the morning. By now you are on two hourly checks. Not really much better. She cleaned me up, got me out of bed, said come on my rounds with me and I had a long walk which I badly needed. I had slept in a chair now for three nights, had about six tubes and drains coming out of me, hadn't showered since the op, my dressing was falling off my breast. I was extremely uncomfortable and this was what I needed to do. I turned the corner that morning.
On the fifth day, drains and tubes started decreasing. I begged the nurse to take off the thigh high compression stockings. They were cutting into me badly. Dr Bayley pointed out to me the error of my ways. Now the big one, my bowels moved that night. Not pleasant but necessary. I had a sore back for two days after this mammoth effort.
Sixth day, I was nearly tube free. One drain left in, I think. My stomach wound had been cleaned and redressed. I had my bra to wear. Dr Bayley came in and said, "What are you going to do today?" "Go home I hope". "I don't see any reason why not." Yes. I rang my husband, had final drains removed, had that first wonderful shower after the op and washed my hair and home I came.
It was great. Now mind you I wasn't fully recovered. Still a way to go. Our bedroom is upstairs - 14 of them - but that was OK. I could get around. Got tired very quickly. Walked bent over for a while. Slept with the help of some pain meds - just Panadol. But could make a coffee, check my emails, do some basic housework. Christmas was fine. I actually made one of the courses for 15 people.
Yesterday I had my final appointment with the plastic surgeon. He was happy with my development. I have a flat stomach. Two boobs that look exactly the same.
Would I do it again? I would think long and hard but probably no. I am glad I did it though as the end result was fabulous. Now I was fortunate - no infections, wonderful support, great doctor, excellent result. But this is my story of having the diep flap. I hope yours is similar. Good luck.

I nearly had an appointment with him tomorrow too. He did the CT angiogram on me too but it had to change once he was in there. Hope it goes well,

Hi Diane,
Yes I'm done, its two weeks today since my op. I came home on Day 8 from Mac Uni Hospital. Doc Penn has done an awesome job. In the end, the plan was to do DIEP on the left (replacing the expander) and a right breast reduction (I was going to have a mastectomy and then an implant but changed my mind). ONce he was there it changed slightly. The best blood vessel was under a previous appendix scar so he had to use other b v in my abdomen. These were separated by muscle so in the end he had to remove some muscle with these b v, so I actually had a TRAM. He did say that was a possibility and that he wouldnt know what would be needed until he was inside me. He replaced it with mesh.
my new boob looks amazing! Soooo much better than the expander. There is a small circle in the middle which is my tummy skin and thats the only skin I can see thats new. i already had 3D shape cos of the expander and he filled in behind. He also lengthened a scar under my arm to access blood vessels and that is quite tender. I also have a strange sensation down the back of that arm and I think perhaps that might be nerves. Hopefully itll settle down.
He reduced my right breast and looks great although heaps smaller than before. It takes some getting used to.
Another lady I met who had this surgery with Dr Penn in may this year was raving about her results but one thing she said that I agree with is that his bedside manner leaves a little to be desired. He visits between 6.30am and 7.30am and perhaps because he's up early he was a bit gruff. Nurses agreed and said that he is a perfectionist but a bit impatient to staff and patients. That said though, I saw him last Friday to have stitches and dressings removed and he was great. Had a good chat, much better than the hospital.
I havent taken any pain killers since I got home. Parts are tender if I bump or lie on them but if I am sitting down I don't feel sore. It's a bit hard to stand up straight (but easier since the drssings were taken off).
My surgery was 10 hrs, its pretty long. When I came out I had 6 drains, a catheter, a drip and a PCA pain control, and a binder around my abdomen. A word of advice about the binder. I developed blisters above it where it rubbed and they were really sore. I also developed an itchy painful rash across my back from the sweaty skin rubbing on the binder (It's pretty rough). If I'd known this was a possibility (and nurses said some but not all people get it), then I would have worn a singlet underneath the binder. Once I did that it was fine and the rash settled down. My interim measure until my husband brought the singlets was to fold a pillowcase underneath the rash.
Three drains were removed on day 3 then two more a couple of days later. ITs not very pleasant to have them pulled out so take pain killers before if you can.

Counting down now to my Diep. How did your op go Heather? How long were you in Hospital? I hope it all went well and you are home now and relatively pain free. I'm keen to hear details as my op is in August.

- DeeayMemberIm having a double DIEP. He also said I didn't have enough tummy fat for two C cups but he will do as big as he can. I was surprised as I have a big roll! I would feel confident in him Heather. Trust him. You probably need skin on your RHS so the implant can go in under. Otherwise you have to have expanders put in and your skin stretched over a few months until it is stretched enough to hold the implant. This way its all donenin the one op. Have you seen Dr Ps website? He is very good at making boobs look the same and he did tell me he does 'tweaks' free of charge for 3? months after surgery until you are happy. I don't need to see him until two weeks before surgery when I have to have the CT scan so he can see where the arteries are he will use. Dr P told me the scar will be above the pubic area and as low as possible (below bikini line) but it depends on previous scarring how high it will be. I've had my gall bladder out the old way so have an incision below my belly on the RHS that he has to consider. I think my scar may be higher. Did you have a Caesarian or hysterectomy? I believe they use that scar line if you have one. As far as scarring on the boobs, he uses the mastectomy scar and then makes a join above that on the top of your boob, but it is below your bra line. That is the tissue and fat from the stomach joined on. He fills in the hollow that is in your shoulder area. I didn't realize Bret tissue went so high until I had a mastectomy and just felt 'scooped out'! Happy to chat more by PM if you like as I'm keen to stay in touch and see how you go. Diane

Hi Deeay,
I made my appt in Feb for July at Macquarie Hosp. I felt very rushed last Friday when I met him for the second time, but I do know that he is good. I think I have to trust him that he knows best.
I have had a mastectomy and chemo and radio on the Left. He will reconstruct this one with DIEP but said I do not have enough tummy fat to do the other side (I am having a mastectomy here too cos there is a strong family history of BC and I'd like my risk to be zero), so I need a silicone implant on this side. He seems to think it will match well. I am worried about where my scars will be and worried about his need to implant skin too. I thought it would be just the skin I already have with my own tissue behind it.
What are you having? Where did he say the scar would be? I thought it would be low down below undie line but apparently its a bit higher than that.
I'm feeling a bit anxious about it all.

- DeeayMemberI'm going with Dr Pennington too. First available date was August! I saw him on 28th March but he didn't have an assistant at Macquarie Hospital after the end of June so couldn't guarantee a date there. I went with Strathfield to have a definite date. Where will you be? He's wonderful isn't he! Are you having a bi lateral DIEP?
