Chances of phase 2 of DIEP flap reconstruction

Hi all, I am considering doing Diep flap reconstruction after a Mascetomey tissue expanders. But I've heard from other chats that women have had to do "phase 2" of diep flap reconstruction,  my PS hasn't mentioned this. I was under the assumption that if all goes well, that it was only one surgery to transfer tissue to reconstruct the breasts. I am feeling a bit anxious with the chances of having to do more than one surgery and thinking of implants now. I'd love to hear from any women who've had diep flap reconstruction and implant reconstructions. Thanks 😊

  • You don’t mention timings of surgery & whether it’s nipple saving/sparing so not sure if this is relevant - I chose DIEP after a single mastectomy with tissue expander was required. I was able to keep my nipples but I had needed  radiation, had an uncommon hematoma (exercised too hard too quickly - dug 2 plants in garden week 4) and I chose to have the DIEP earlier than most (7mths post SMX, just wanting expander out/moving most of treatment into a single financial year to max Medicare safety net), so I’m needing a revision. Main reasons are my cup size doesn’t match due to radiated high tight breast vs natural, have 2 different colour nipples (nipple tan is real!) and most importantly they will be tidying up my scars - I have a dog ear scar on my hip, an adhesion from the 2nd breast surgery (causing nerve pain becoz of how it was attached to side of breast) & again maybe relevant to you/anyone keeping nipples, there’s an incision under breast that needs to be redone from where they monitor the health of the tissue week after surgery (I didn’t realise what this would look like - think of it as a deep pocket of skin once healed. Revision will be much cheaper than initial surgery, my PS claims next to never is a 3rd required unless fat doesn’t take, and bear in mind my situation may not be the norm - he says <50% have revision but often told it’s often due to cost or that they’re tired of surgeries. I am waiting longer to do it to have best chance of only doing the 1 revision - day/overnight procedure, faster recovery but wearing wider compression gear for 6weeks as breasts and lipo sites need it.
