Results today. Back to surgery. They missed the nodes??

Wanted to post today to share what happened with my results.
Surgery was 10 days ago, recovery been up and down. Still ill off and on with breast discomfort and mild temps and aches and pains, still on antibitotics.
The results were the tumour was taken out, it had pre cancer cells on the outer part, but the margins they took left a clear area of margin, so that was great news.
It came back as Stage 2 cancer, border line they said.
There was a lot of talk about seeing the chemo team to discuss my options.
First though they are sending me back in for surgery in next 10 days as apparently the clump of nodes they took contained no nodes. At the time they told me the dye didn't travel to the main sentinel node properly so they just took a clump. That clump ended up being fatty tissue and no nodes.
So was definitely not what I expected to hear. I had it in my mind Stage 1, Clear Margins, Clear Nodes, Radiation and all done.
Now I'm not sure. Once I have surgery and the do the Axillery Node Disection, Level 2, then they will know if they are clear etc.
Still a bit confused though with the chemo, so I have a couple of questions.
1. Does a Stage 2 cancer normally need chemo as well as radiation, hormone therapy (they said I would have the hormone treatment and radiation for sure)
2. Has anyone else gone in and come out with no nodes taken, completely missed?
3. And has anyone had same cancer as me, invasive ductal carcinoma grade 2 and had chemo or refused it?
