Like all the ladies here I struggled with this decision too.
In the end I followed my surgeons advice do one breast as in theory the other was fine. I did have concerns though as my breast tissue is extremely dense and hard to monitor. I raised this but was reassured this would be fine.
I then struggled with LD or implant choice but went with the LD. I was worried about it being a bigger operation etc but my doctor felt he could get the most natural symmetry with autologous tissue. A few days before my operation i was still in such turmoil that I returned to my dr to go through it all again.
3 days post op and I was so happy with my choice, the resemblance to my unaffected is really good and I felt sure I'd made the right choice. Then my pathology came back and there was so much more DCIS than they ever expected in the tissue removed during mastectomy.
As a result I've been asked to return for 3 random biopsies on right " unaffected" breast because on mamo, u/s and MRI both breasts appear similar except for some initial calcification in the originally affected breast.
So my advice is.if you decide to remove one breast only and you have very dense breasts you want a pretty good explanation of what's going to be involved in monitoring.
Dense breasts are more difficult to screen.
